Patient Portal Access

When you become a patient at La Vie Aesthetics & Wellness, you will be sent a link to sign up for the patient portal. From there, you can upload any required documentation, fill out all your paperwork, message the office and schedule appointments with our providers.

Please know that any and all paperwork MUST be filled out prior to your appointment in our office. Failure to complete paperwork prior to your appointment WILL result in a shortened or rescheduled appointment.
