As a La Vie Aesthetics & Wellness client, you understand that our services are provided with the best intent and highest quality measures. While this is not an exact science, many treatments are intended for better health, and outcomes will vary. Any concerns should be brought to the immediate attention of Dr. Smaldone. If they are not, we are in no way responsible or held liable. A clinical solution will be agreed upon, and no further action will be taken beyond that of the client-provider relationship.

The client also agrees to be photographed for clinical results and educational and promotional purposes. The least identifying information will be used, and all images will be discrete. The client also agrees to the transmission of images and information via email and text for clinical purposes.

If you are calling to refill a medication, please submit your request through our Tebra secure messaging or call the office and press option 3 for our medication mailbox. By leaving your medication request, this is an authorization to charge your card on file. If you do not have one on file, we will reach out to you. In order for us to order your medication, payment must be collected PRIOR to ordering the medication. We will send you a message via tebra or email to inform you of the price being charged to the card on file. If you DO NOT wish to have a card on file, you MUST prepay for all visits, medications, products, treatments, etc. PRIOR to ordering or scheduling with the office.

We kindly ask for at least a week in advance when ordering your medications. If you are calling for injections and wish to change the dosage, have questions or want to talk about switching to another medication, please call the week before your scheduled pick-up.

If you are unable to pick up your injections at your SCHEDULED time, we are happy to hold them until the following Monday after your appointment. If you do not pick them up by the following Monday, you will be charged a $50 medication drawing fee to your card on file or it will be added to your next visit total. Please come at your scheduled pick-up time or call us if you will be late. This will allow for a seamless pick up in the office.

Procedure appointments and consultations/follow-up appointments require a nonrefundable $50 booking fee and filler appointments require a nonrefundable $100 booking fee. Booking fees will be deducted from your treatment/visit total if the appointment is attended. If you miss the appointment or cancel, the booking fee will not be refunded. If you need to cancel the appointment/procedure, please call at least 48 hours before your scheduled appointment.

Consultations with Dr. Smaldone are $250 and follow-up appointments are $150 (both require a $50 nonrefundable booking fee). Nurse visits are $100 and require a $50 nonrefundable booking fee.

Follow-up appointments and bloodwork MUST be completed every 6 months in order to remain on your prescribed medication (Hormone replacement and weight loss).
